Why Oestradiol (E2) matters
Oestradiol is the primary oestrogen and the most biologically active form, present in both men and women. In men, oestradiol is produced through the peripheral conversion of testosterone via aromatase (found in fat tissue). In women, oestradiol is produced primarily by the ovaries and supports bone health, cardiovascular function, cognitive function, and reproductive physiology. In men, oestradiol is necessary for bone health and sexual function at physiological levels; excessive conversion to oestradiol can impair testosterone-driven adaptations.
The range explained
Low levels
Low oestradiol in women (below 75 pmol/L outside the follicular phase) may indicate insufficient ovarian function or nutritional deficiency. In men, low oestradiol is rare and may affect bone health. Interpretation depends on sex, menstrual cycle phase, and other hormonal markers.
Normal range
Reference ranges vary widely by sex and cycle phase: women 75–400 pmol/L (follicular phase), men 40–160 pmol/L. These represent physiological ranges within which reproductive and metabolic health are typically supported.
High levels
Elevated oestradiol in men (above 160 pmol/L) may reduce testosterone-driven muscle growth and increase water retention. In women, consistently high levels may warrant investigation. Can be related to excess body fat (which increases aromatase activity) or liver function.
What affects Oestradiol (E2)
- Training: Resistance training increases aromatase activity, promoting oestradiol conversion from testosterone in men. Excessive endurance training without strength work can elevate aromatase.
- Nutrition: Excess body fat increases aromatase activity and oestradiol production. Cruciferous vegetables and fibre support oestrogen metabolism. Alcohol impairs liver processing of oestrogens.
- Sleep: Sleep deprivation impairs oestrogen metabolism and can elevate oestradiol. Consistent sleep supports healthy oestrogen balance.
- Stress: Chronic stress alters the oestrogen-to-testosterone balance. Stress reduction supports hormonal balance.
When to retest
In women, testing at a consistent cycle phase (usually day 21 for luteal phase) is important. Every 12 weeks for athletes monitoring hormones; every 12 months for general health.

In men, athletes monitor oestradiol because excessive conversion from testosterone to oestradiol can reduce net muscle-building stimulus. Maintaining balanced oestradiol (not too high or low) supports both muscle and bone health.
