Why ALP matters
Alkaline phosphatase (ALP) is an enzyme found in liver, bone, kidney, and intestines. It is used clinically as a marker of bone metabolism and liver function. ALP is elevated during periods of bone growth or remodeling (adolescence, recovery from fractures). In athletes, ALP may be monitored as part of assessing bone health and recovery, particularly in strength athletes who place high mechanical loads on bone.
The range explained
Low levels
Low ALP is not typically clinically significant, though very low levels may indicate hypophosphatasia (a rare genetic condition).
Normal range
Reference range: 30–120 U/L (note: higher in children and adolescents due to bone growth). This represents normal liver and bone metabolism.
High levels
Elevated ALP (above 120 U/L) can indicate bone remodeling, liver disease, or growth. In athletes, mild elevation may reflect bone adaptation to training.
What affects ALP
- Training: Strength training and mechanical loading stimulate bone remodeling and can elevate ALP as part of bone adaptation.
- Nutrition: Calcium, vitamin D, and phosphate support bone metabolism and ALP. Deficiency impairs bone health.
- Sleep: Sleep supports bone remodeling during recovery.
- Stress: Chronic stress can impair bone metabolism.
When to retest
Every 12 months for general health.
Why athletes care
Strength athletes may see mild ALP elevation as part of bone adaptation to training; sustained high ALP warrants investigation.
