Why ALT matters
Alanine aminotransferase (ALT) is an enzyme found primarily in liver and also in muscle and other tissues. ALT is a more liver-specific enzyme than AST; elevated ALT is typically associated with hepatic (liver) injury or inflammation. In athletes, ALT can be transiently elevated after intense training but is generally more stable than AST. ALT is best interpreted alongside AST and the AST:ALT ratio, which helps distinguish between liver injury and muscle damage.
The range explained
Low levels
Low ALT is not clinically significant.
Normal range
Reference range: 7–45 U/L. This represents normal liver enzyme levels.
High levels
Elevated ALT (above 45 U/L) typically indicates hepatic injury or inflammation. In athletes, mild elevation is less common than AST elevation post-training.
What affects ALT
- Training: ALT is less affected by training than AST. Sustained elevation suggests liver stress rather than muscle breakdown.
- Nutrition: Alcohol is a primary driver of elevated ALT. Adequate nutrition supports liver health.
- Sleep: Sleep supports hepatic function.
- Stress: Chronic stress and poor metabolic health can elevate ALT.
When to retest
Every 12 months for general health; more frequently if elevated.
Why athletes care
Athletes monitor ALT as a marker of liver health; sustained elevation indicates liver stress and warrants investigation.
