Why Prolactin matters

Prolactin is a pituitary hormone with primary roles in breast tissue development and milk production in women. In both men and women, prolactin has additional effects on the immune system, mood, and sexual function. Elevated prolactin can suppress gonadotropin-releasing hormone (GnRH) and reduce sex hormone production in both sexes, making it relevant to reproductive health. Prolactin is released in response to several stimuli, including stress, sleep, and nipple stimulation.

The range explained

Low levels

Low prolactin (below 5 mIU/L in men and non-lactating women) is rare and may indicate pituitary dysfunction. Clinically, low prolactin is not typically concerning.

Normal range

Reference ranges: men 3–13 mIU/L, non-lactating women 3–29 mIU/L. Lactating women have significantly higher levels. These represent physiological baseline levels.

High levels

Elevated prolactin (above 13 mIU/L in men, above 29 mIU/L in non-lactating women) may indicate a prolactin-secreting pituitary adenoma, hypothyroidism, or medication effects. High prolactin can suppress testosterone and ovulation.

What affects Prolactin

  • Training: Intense or prolonged exercise can transiently elevate prolactin. Recovery supports normalization.
  • Nutrition: Adequate energy and protein support hormonal balance including prolactin. Underfueling can elevate prolactin.
  • Sleep: Prolactin is released during sleep. Poor sleep may dysregulate prolactin secretion.
  • Stress: Psychological stress elevates prolactin through TRH (thyrotropin-releasing hormone). Stress management supports normal prolactin.

When to retest

Every 12 months for general health; more frequently if elevated.

Why athletes care

Athletes monitor prolactin because elevated prolactin can suppress testosterone and LH, impairing recovery and performance. Prolactin elevation often accompanies overtraining and insufficient sleep.
